Entry form for 2025 Holiday Dance Showcase. Good for "Solos" which is either solo dancer or a couple. For Formation groups please use paper form.
TERMS AND CONDITIONS:
Date and Time: 6:30 to 10:30 p.m. on Saturday, December 20, 2025. Ending time is approximate.
Rehearsal: You may rehearse on the fully set up floor between 11 a.m. and 2 p.m. on Saturday, December 20, 2025.
Liability Disclaimer: Each person signing this form agrees to release DanceSport Club, and any and all instructors or personnel connected with the production of this show from any and all claims due to injury to person or property while working on or participating in this show.
Videography: DanceSport Club, will film the show and offer DVD’s for sale to both participants and the general public. No other video equipment will be permitted in the ballroom during the show. Each person signing this form agrees to permit DanceSport Club, to film all show performances for this stated purpose
Note about tickets: Each participant, including teachers, must purchase a ticket for his or her own seat
